Homebound Process

Things to Note:
  • Homebound Services DO NOT carry over from last school year. 
  • Homebound Instructors must be a Licensed Teacher
  • Homebound Start/End Dates are based solely on the timeline specified by the referring physician, not when the paperwork is submitted to the school.
  • Homebound Packets will not be submitted for approval until ALL required pieces have been received.

Next Steps:
  1. Submit all required documents to Jamie Milam in the Student Services Department. 
  2. Ask your admin to submit a Homebound Teacher Recommendation in Frontline. 
  3. Wait for the notice of approval. Once approved, services can begin.  

When a student returns from homebound - please complete the Homebound Exit form.
