Work Injury Information and Forms

First Report of Injury - This form MUST be completed and sent in immediately following an injury being reported to the supervisor on duty, regardless of the employee's intention to go to Urgent Care or seek medical attention for the injury.

Incident Investigation Form - This form must also be completed by the supervisor and accompany the First Report of Injury when sent to Human Resources.
